A Resource Table is included with each level of sponsorship of organizations that provide services for substance use/mental health/wellness.

By submitting this form, I affirm I am duly authorized to execute this agreement on behalf of the organization and agree to comply with the rules and guidelines of Recovery Rocks, an event hosted by The Mom of an Addict, Inc., as well as those of Fort Wayne Parks and Recreation and all other applicable laws. I acknowledge photos and videos will be taken before, during and after the event, and I grant to The Mom of an Addict, Inc., their assigns, licensees and successors the right to use my and our organization representatives present at the event’s image in all forms and media. This shall include recaps of this year’s event and promotion of the event in subsequent years. I/we waive the right to inspect or approve versions of said images used for publication or the written copy that may be used in connection with the images.
